Good incident records are structured, timely and boring. Here is the minimum standard every home should hold itself to.
Structure beats prose
Free-text incident notes are hard to audit and easy to write inconsistently. Capture type, severity, location, time, actions taken and notifications as discrete fields, then let the narrative add context on top.
Structured data is what allows you to see that eight of eleven falls this quarter happened in the same corridor after 8pm.
Document within the shift
Memory decays fast and shift-end reporting is where detail is lost. Documenting before the shift ends is the single largest quality improvement most homes can make without hiring anyone.
Close the loop with families
Record who was notified, when, and by whom. Families rarely escalate because something happened; they escalate because nobody told them, or the story changed between callers.
